Your Insurance

Many insurance companies for behavioral health require the subscriber/patient to contact their office before the first appointment to acquire an authorization number for treatment.  If this is not done many insurance companies will deny coverage for treatment.  This is a call that you the subscriber can make.  There is a place on the Patient Information form for you to enter this authorization number. Most insurance companies require the patient to pay a "copay" amount, and some require you to meet a "yearly deductible" amount.  Payment of your copay amount is due at the beginning of each session.  

Office Policies

Please be sure to read in detail all contents of the form that discusses our office policy.  This document covers many topics that are important, and is meant to  provide  important information about our professional services.  If you have any questions or concerns about anything included in this  document, please discuss them with the therapist  either before or during your first appointment.

Parking

When you arrive at our office, enter the driveway on the right side of the building and proceed to the parking lot in the back.  The main entrance is off of the parking lot.  Enter through the French door, and make yourself comfortable in the waiting room.  Your therapist will come out to the waiting area to greet you at your appointment time.

Click on the link above for "Office Forms", and then select the therapist that you will be meeting with.  Print out the forms, and then bring the completed forms and your insurance card to your first appointment.
